These are two very similar medications that are in the glucocorticoid class of medications. Many man made and semi-synthetic medications are in the class called glucocorticoids. Two of these are prednisone and prednisolone. Prednisone is normally used as an by mouth medication. It is stronger milligram for milligram than cortisol. One mg. of prednisone has a comparable capability as 5 mg of cortisol. The adrenal gland naturally releases about 20 mg of cortisol into the system daily. In patients with adrenal insufficiency a typical daily amount of prednisone to be used for hormone replacement therapy is 5 mg daily. Prednisone is not soluble in water, and so can’t be given intravenously. Prednisone taken orally is metabolized into prednisolone by the body processes.
Prednisolone is also available as a medication. It is soluble in water, and so can be given intravenously, as well as being injected into tissue and joint spaces. Prednisolone is a popular INTRAVENOUS glucocorticoid, and is particularly sometimes given orally. It is a common by mouth preparation for children, as it is soluble and so can be wiped out in water and so changed to syrup. Prednisolone is slightly stronger on a mg. for mg. basis than prednisone. About 4 mg or prednisolone is equivalent to 4 mg of prednisone and 20 mg of cortisol.
